What's happening?
Nicotine stains on a ceiling are a film of tar and nicotine deposited from tobacco smoke. Unlike a single water tide mark, the stain is often even across the whole ceiling and high on walls. Water-based emulsion re-dissolves that film, so more coats without a barrier bring the yellow back. The fix is wash-down, a stain-blocking primer suited to nicotine, then a full recoat so sealed and unsealed areas do not flash against each other.
What usually causes it?

Yes on a dry, sound surface: 1. Wash with sugar soap or a suitable cleaner and rinse; let dry fully. 2. Apply a stain-blocking primer over the whole stained area (often the full ceiling).
